Description & Services
Description (Service) Offers assistance to patients who are transitioning from hospital to home through the following services: 
  • Transportation home 
  • Assistance with picking up prescriptions 
  • Light housekeeping 
  • Meal preparation 
  • Referral to community support services
Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Schumacher ; South Porcupine ; Timmins
Eligibility Ages: 55 year(s) and up 
 
  • Be able to direct own care or caregiver;  
  • Be medically stable; 
  • Be able to be left unattended; 
  • Requires one or both the transportation and settling in component of the program; 
  • Be mobile, ambulatory (able to self-transfer), and not need the assistance of more than one person to manage stairs; 
  • Pose no threat to the health and safety of self or workers; AND 
  • Not live in a long-term care home or Assisted Living where 24-hour care is provided
Application Potential clients are identified by the hospital or via the Community Care Access Centre (CCAC) and must be received a minimum of 24 hours in advance
