Description (Service)
  • Provides a mental health program consisting of a psychiatrist and a mental health nurse clinician 
  • Offers a consultation service where a senior is first assessed to determine their needs and a consultation report is completed and forwarded to the referring family doctor or nurse practitioner
Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Cochrane ; Timiskaming District
Eligibility Ages: 65 year(s) and up 
 
Individuals experiencing an age-related mental illness
 
Individuals younger than 65 also eligible if they meet one or more of following criteria: 
  • Mental health problems complicated by age-related medical illness/functional needs 
  • Behavioural problems associated with dementia and/or other severe mental illness 
  • Must be medically stable (e.g. not in a medical crisis) 
  • Must reside in the community (living at home, supportive housing or in a facility based  
    environment such as a long term or chronic care)
