Description (Service) Facilitates the following patient self-management program which was developed through the Chronic Disease Self-Management Program 
  • Provides a six week program to help manage symptoms and treatment side-effects 
  • Assists with setting goals and solving problems 
  • Teaches strategies to manage medications and emotions 
  • Provides nutrition, diet and exercise information 
  • Includes working and communicating with a healthcare team
Hours Mon-Fri 9 am-5 pm
Areas Served Cochrane ; Englehart ; Hearst ; Hornepayne ; Iroquois Falls ; Kapuskasing ; Kirkland Lake ; Matheson ; Smooth Rock Falls ; Timmins
Eligibility Post treatment cancer survivors and caregivers
