Description (Service) Provides a variety of community based services to improve, maintain and/or restore stability to an individual living in the community
  • Provides ongoing assessments of independent functioning, daily living skills, and interpersonal skills 
  • Assists individuals with the development of personal goals 
  • Offers assistance with problem solving and personal development 
  • Teaches independent living, functional, social and leisure skills 
  • Coordinates services provided by other agencies and professionals 
  • Provides support to individuals experiencing a mental health crisis
Case Management 
  • Offers individuals the opportunity to participate in an intense recovery process focusing on changing attitudes, values, and feelings
Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Cochrane District ; Timiskaming District
Eligibility Individuals with a serious mental illness (e.g. main diagnoses of schizophrenia or bipolar disorder)
Case Management 
  • Individuals receiving Community Support Services (services are designed to enhance and complement Community Support Services)
Application Referrals are accepted from anyone but the individual must voluntarily accept and desire services
