Description (Service)
  • Provides clinical services on an individual, family, and group basis 
  • Offers a variety of services aimed at helping the individual and his/her family to cope and adjust to a new diagnosis and/or acquired trauma 
  • Focus of involvement is often related to the impact of crisis, disability and hospitalization on the family 
  • Focus may also be related to other psycho-social needs or problems identified by the family such as finances, lack of support services, and mental health of family members
Clinical service interventions can include: 
  • Advocacy 
  • Bereavement/grief/work denial/adjustment 
  • Coordination/community liaison/practical supports and facilitator 
  • Counseling 
  • Crisis intervention 
  • Educating families and individuals re: community services and supports 
  • Family/Individual assessment 
  • Transitional planning
Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Cochrane District ; Timiskaming District
Eligibility Ages: 18 year(s) and under 
Children living with disabilities/physical impairments or have experienced acquired trauma and their famiies
Application Must be referred by CTC staff only
