Description (Service)
  • Provides young adults with a smooth and successful transition from high school to adult life 
  • Develops and implements a Person Centered Plan (PCP) 
  • Offers an outcome-oriented approach addressing future vocational goals, career and volunteer placements, funding sources, leisure and recreational opportunities and community resources 
  • Focuses on being out in the community and providing the opportunity to explore choices based on what has been identified in planning
Hours Mon-Fri 8:30 am-4 pm

Eligibility Ages: 18 year(s) - 27 year(s) 
 
  • Individuals with an intellectual disability 
  • Individuals exiting high school and require support to plan for the next phase 
    of life 
  • Individuals must not be receiving any traditional day supports
