Description (Service) Provides an integrated and coordinated approach to diabetes treatment, management and support 
 
Offers a variety of programs and services including: 
  • Individual assessment, individual outcome monitoring and follow-up support 
  • Educational workshops and group programs 
  • Foot care clinics 
  • Referrals to other health centre programs and specialists
Hours Mon-Fri 8:30 am-12 noon, 1 pm-4:30 pm
Areas Served Greater Sudbury ; Henvey Inlet First Nation ; Magnetawan First Nation ; Wahnapitae First Nation
Eligibility Ages: 19 year(s) and up 
 
Indigenous individuals and families who are living with diabetes or at risk
