Description (Service) Offers both short and long term supportive practical assistance such as: 
  • Food enhancements and access to Food Pantry 
  • Transportation to medical appointments 
  • Home, hospital, or jail visitations and support 
  • Assistance accessing drug benefits
Hours Mon-Fri 9 am-5 pm
Areas Served Greater Sudbury
Eligibility
  • Individuals living with HIV/AIDS and/or Hepatitis C 
  • Proof of HIV and/or Hepatitis C status is required and can be verified through a copy of the serology blood work 
  • Applicants need to demonstrate an active involvement with their continuum of care by actively participating to the Agency's programs and other health care services within the community
