Public Comments For individuals who have computer and printer access, a printable form can be downloaded and filled out to bring to the appointment. If not, the form is available for individuals when they come for their appointment.
Description (Service) Provides mental health services for mothers during pregnancy and up to 12 months after the baby is born
 
Services provided include: 
  • Family assessment and support 
  • Individual assessment and treatment 
  • referral to community resources
Hours Central Intake and Referral: Mon-Fri 8:30 am-4:30 pm

Eligibility Ages: 16 year(s) and up 
 
For mothers who are experiencing depression and anxiety during pregnancy and up to 12 months after the baby is born
Application Self-referral, referral from community agencies, family doctor, obstetrician, midwife, and mental health professionals or through Central Intake and Referral at 705-523-4988 ext 4221
