Description (Service) Provides case management and clinical services and supports to assist individuals to achieve and maintain a quality life
  • Assists individuals in developing a realistic plan to acquire the skills and supports to enable them to live independently 
  • Determines areas of need and develops the support plan with a focus on each individual  
  • Provides referrals to community resources and supports  
  • Provides the supports in a community setting or within an individual's home environment 
  • Focuses on wellness and recovery
Supports can include assistance in the following areas: 
  • Daily living skills 
  • Employment 
  • Social skills 
  • Navigating the mental health system 
  • Accessing medical and psychiatric services 
  • Crisis prevention, intervention and management
Hours Mon-Tue, Thu-Fri 8:30 am-4:30 pm * Wed 8:30 am-6 pm
Areas Served Manitoulin District ; Sudbury District
Eligibility Ages: 16 year(s) and up 
Must have a mental health diagnosis or are experiencing symptoms of a mental illness
