Description (Service) Provides health care services for ill, injured or elderly community members in their own home
 
Provides the following services through a case manager/nurse and a personal support worker: 
  • Assessment to determine individuals health history and develop a personalized care plan 
  • Case management to ensure that care plans are being carried out and that the plan developed is still suitable  
  • Home care nursing duties 
  • Personal care help with activities such as dressing and bathing 
  • Basic foot care 
  • Teaching the individual in need to care for themselves 
  • Home support with light housekeeping, laundry and meal preparation 
  • Respite for family members and caregivers 
  • Help with light outdoor/yardwork
 
Meals on Moccasins 
  • Provides full-course meals and delivery to individuals in their homes 3 days per week
Hours Mon-Fri 8:30 am-4:30 pm, Sep-Apr * Mon-Thu 8 am-5 pm, Fri 8 am-12 noon, May-Aug 
Meals on Moccasins: Wed-Thu at supper hour

Eligibility Members of Chapleau Cree First Nation requiring assistance at home due to illness, injury, pregnancy or age (55 years of age and older)

  • Assessment required for program enrollment
 
Meals on Moccasins 
  • Must order meals at least 1 week prior to delivery 
  • Must cancel before 11am day of delivery to avoid charges
