Public Comments Diabetes Care team consists of a Diabetes Nurse Educator, a Registered Dietitian and a part-time Foot Care Specialist
Description (Service) Provides diabetes education, prevention and management services to communities in the North Shore Tribal area
 
Minomodzawin Diabetes Education 
  • Provides workshops, seminars and events in communities to promote good nutrition and active lifestyles 
  • Hosts food booth displays, healthy lifestyle choices and traditional health teachings workshops, youth sports camps and training sessions and various diabetes conferences
 
Naadmaadsaan Diabetes Education
 
Funded by the Northern Diabetes Health Network, this program focuses on preventing complications by delivering: 
  • Education in self-management 
  • Individual or group counselling sessions 
  • Individual assessments and follow-up support 
  • Nutritional education and support 
  • Basic foot care services
Hours Office Hours: Mon-Fri 8:30 am-4:30 pm
Areas Served Atikameksheng Anishnawbek First Nation ; Batchewana First Nation ; Garden River First Nation ; Mississauga First Nation ; Sagamok Anishnawbek First Nation - ,, , ; Serpent River First Nation ; Thessalon First Nation
Eligibility Must belong to one of the member North Shore Tribal First Nations
Application Services are delivered to community members at the local level (Health Centre)
