Public Comments Diabetes Care team consists of a Diabetes Nurse Educator, a Registered Dietitian and a part-time Foot Care Specialist
Description (Service) Provides diabetes education, prevention and management services to communities in the North Shore Tribal area
 
Minomodzawin Diabetes Program 
  • Provides health promotion and diabetes prevention activities 
  • Provides education sessions through workshops, seminars and events 
  • Offers individual and group counselling including nutrition counselling 
  • Conducts individual assessments and follow-up support
 
Naadmaadsaan Diabetes Education 
  • Provides education in diabetes self-management 
  • Offers individual or group counselling sessions 
  • Conducts individual assessments and follow-up support 
  • Offers nutritional education and support 
  • Provides referrals to foot care services 
  • Collaborates with clinic outreach services for high-risk individuals
Hours Office Hours: Mon-Fri 8:30 am-12 noon, 12:30 pm-4 pm
Areas Served Atikameksheng Anishnawbek First Nation ; Batchewana First Nation ; Garden River First Nation ; Mississauga First Nation ; Sagamok Anishnawbek First Nation ; Serpent River First Nation ; Thessalon First Nation ; Urban Indigenous population in Sault Ste Marie
Boundaries Naughton to Sault Ste Marie (Atikameksheng Anishnawbek to Indigenous Friendship Centre - Sault Ste Marie)
Eligibility Members of a First Nation community or part of the urban population of Sault Ste Marie belonging to the North Shore Tribal Council living with diabetes or at-risk
