Description & Services
Description (Service) Advises and assists seniors and their families and disabled individuals leaving the hospital to navigate through the community and health care system to access needed health services
  • Conducts a hospital visit to meet with individual prior to discharge to determine what helps are required to ensure continued care upon returning home 
  • Liaises with hospital care team to address health care needs of individual 
  • Provides assistance to access the necessary services
Provides information and links with: 
  • North East Local Health Integration Network (NELHIN) 
  • First Nation community support services 
  • Non-Insured Health Benefits 
  • Primary Care services
Services include: 
  • Assisted living services for high-risk seniors 
  • Falls prevention 
  • Occupational therapy 
  • Physiotherapy
Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Atikameksheng Anishnawbek First Nation ; Batchewana First Nation ; Garden River First Nation ; Mississauga First Nation ; Sagamok Anishnawbek First Nation ; Serpent River First Nation ; Thessalon First Nation
Eligibility Members of a First Nation community or part of the urban population of Sault Ste Marie belonging to the North Shore Tribal Council who are 55 years of age or older or chronically disabled
Application Services are delivered within one of the area hospitals and in-home
Languages English ; Interpretive Services - Ojibway, available upon request
