Description & Services
Description (Service) Provides supports and services for the elderly and individuals living with a disability 
  • Works on development of community and regional infrastructure 
  • Coordinates advocacy, training and program planning
 
Services include: 
  • Management of a loan cupboard 
  • Hospital discharge planning to address health care needs, and to help access services necessary to ensure continued care upon return home 
  • Community based physiotherapy, and occupational therapy 
  • Rehabilitation assistance 
  • Assisted living services for high-risk seniors 
  • Personal support care for individuals who require temporary services while waiting for long-term care placement, palliative care, convalescence care, or who would otherwise occupy an alternate level of care bed in hospital 
  • Arthritis management program
Hours Mon-Fri 8:30 am-12 noon, 12:30 pm-4 pm
Areas Served Atikameksheng Anishnawbek First Nation ; Batchewana First Nation ; Garden River First Nation ; Mississauga First Nation ; Sagamok Anishnawbek First Nation ; Serpent River First Nation ; Thessalon First Nation ; Urban Indigenous population in Sault Ste Marie
Boundaries Naughton to Sault Ste Marie (Atikameksheng Anishnawbek to Indigenous Friendship Centre - Sault Ste Marie)
Eligibility Members of a First Nation community or part of the urban population of Sault Ste Marie belonging to the North Shore Tribal Council who are 55 years of age or older or chronically disabled
Application Services are delivered within one of the area hospitals and in-home
Languages English ; Interpretive Services - Ojibway, available upon request
