Description (Service) Provides education and services for the prevention and management of diabetes 
  • Offers services to community members through the North Shore Tribal Council 
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes 
  • Advises on developing and implementing an approach to care and management of diabetes 
  • Assists individuals in finding appropriate services
 
Provides education on diabetes prevention through: 
  • One-on-one counselling 
  • Home visits 
  • Presentations 
  • Workshops
Hours Mon-Fri 8:30 am-12 noon, 1 pm-4 pm

Eligibility Members of Thessalon First Nation with diabetes or at-risk
Application Requires prior arrangements for home visits
Languages English ; Interpretive Services - Ojibway, available upon request
