Hours Mon-Fri 7:30 am-4:30 pm

  • Members of Serpent River First Nation 
  • Infants: children 6-18 months of age 
  • Toddlers: children 18-30 months of age  
  • Preschool: children 30 months - 5 years of age

  • Copy of child's immunization records
